Rethinking healthcare to improve safety
The Victorian Managed Insurance Agency (VMIA) hosted a full day's event at the Victorian State Library in Melbourne on 6th of June 2017. Speakers included:

Stephen Duckett - Grattan Institute Health Program and author of the review of the Djerriwarrh maternity unit discussing Quality and safety of care

James Titcombe - Patient safety specialist and advocate, author of 'Joshua's Story' and father of Joshua Titcombe
'Joshua's Story - Learning from Morecambe Bay'

Nigel Broughton - Orthopaedic surgeon and State committee member for RACS and AOA on 'Influencing surgical practice to improve patient outcomes'

Yvonne Toft - Associate Professor of Human Factors at Central Queensland University  'Socio-technical Systems Safety in Health Care'

Stuart Dickinson (Human Risk Solutions) and Nathan Farrow (Safer Care Victoria) outlining a course sponsored by the VMIA Human Factors Program - Metropolitan Pilot

Stuart Marshall Associate Professor of Medical Education and Anaesthetist, presented a case discussion of a systems error, how to investigate, involve and disclose information to patients and relatives.
